Numbness and Tingling: Which Scan

Pins and needles has three addresses — brain, spine, and nerve. The pattern on your skin draws the map.

Quick Answer

Numbness is imaged by where it lives: a whole-sided face-arm-leg pattern points to the brain (MRI); a band around the torso or a leg-only pattern points to the spine (MRI); a hand-only or foot-only pattern often points to a trapped peripheral nerve (ultrasound or no scan at all). Sudden numbness with weakness or speech trouble is a stroke emergency — CT first, MRI after.

The stroke rule first

Sudden one-sided numbness — face, arm, and leg together — or numbness with slurred speech, vision loss, or imbalance is a stroke until proven otherwise: emergency call, CT to rule out bleeding, and treatment on a clock. Nothing in this article outranks that rule.

The gradual patterns

Gradual one-sided numbness earns brain MRI to look for plaques, small-vessel disease, or structural causes. Numbness in a band around the trunk, or both legs, earns spine MRI — the spinal cord's territory. Both are outpatient-prompt, not emergency, when weeks old and stable.

The glove-and-stocking pattern

Both feet, both hands, symmetric — the classic peripheral-neuropathy pattern, most often diabetes, alcohol, B12, or medication. That is blood tests and nerve studies (EMG), not scanning. Scans of spine or brain here find incidental wear that misleads.

Single-nerve territories

Numbness in one hand's outer three fingers (carpal tunnel) or a shin strip (peroneal nerve) is a trapped-nerve question — clinical exam, sometimes ultrasound or nerve conduction studies. Carpal tunnel rarely needs any imaging.

Frequently asked questions about numbness Imaging

Can a pinched nerve in the neck cause hand numbness?

Yes — a dermatomal hand pattern with neck pain points to the spine MRI.

Is numbness without pain serious?

The pattern matters more than the pain: sudden, one-sided, or progressive patterns all deserve assessment.

What is EMG and when does it replace scans?

Nerve-conduction testing measures the peripheral nerves' function — first-line for the symmetric patterns.

Will MRI find why my foot tingles at night?

Usually that is positional nerve compression — harmless and transient. Persistent tingling in one foot is the neuropathy conversation.

Can anxiety cause numbness and tingling?

Yes — hyperventilation classically causes both-hand tingling around the mouth. It is a diagnosis after the red flags are cleared.

How urgent is new one-sided numbness?

Sudden means emergency. Over days with weakness means prompt clinic. Over weeks, stable, means book a routine workup.
